How often are safety concerns raised about a drug after it's been approved by the FDA? Nicholas Downing, MD, of the Department of Medicine at Brigham and Women's Hospital, and colleagues have found that for drugs approved between 2001 and 2010, nearly 1 in 3 had a postmarket safety event. The team defines postmarket safety events as those that lead to either withdrawal from the market due to safety concerns, a boxed warning or FDA issuance of a safety communication. They found that of 222 novel therapeutics the FDA approved during this time period, three were withdrawn, 61 received boxed warnings and 59 elicited safety communications. The team's findings are published in JAMA.

A father and three children have been winched to safety after spending several hours stranded on Gold Coast waters overnight.The family was heading home from an afternoon out on the water on a jet ski when they took a wrong turn down an estuary and became caught in mangroves north of Sovereign Island.

Federal transportation safety officials headed to Las Vegas to investigate a collision this week between a self-driving shuttle bus on its first day of service and a truck, which was blamed on human error. 
The US National Transportation Safety Board, which has the power to issue safety recommendations and determines probable causes of crashes, wants to learn more about “how self-driving vehicles interact with their environment and the other human-driven vehicles around them,” said NTSB spokesman Christopher O’Neil.
There have been other crashes involving self-driving vehicles but this was the first involving a self-driving vehicle operating in public service, O’Neil said.
